Assignment Sheets

Listed below are trigger/response sheets and responsibility assignment sheets.  The triggers are the designated World Health Organization phases of a Pandemic Influenza.  All of the sheets start with the trigger as the WHO phase 3 because that is where we stand now.  There are trigger/response sheets designated for specific types of groups, but they all start with items to consider for a general business. The lists are not all inclusive but a place to start.  Many items listed may not apply to your business or organization and you may need to add several responses specific to your situation. (Restaurants are different from car dealers etc. so each business or organization needs to tailor their own plan.) 

Most planning should be done during WHO phase 3 as it is likely the time to plan will diminish once the world situation changes and the WHO phases upgrade in urgency to phases 4, 5 and 6.  The definitions for the phases are included on the trigger/response sheets.     

The responsibility Assignment sheet is provided so that the responses or tasks listed can be assigned to someone in your organization. Each page is for one person (with alternates listed).  The assignments are again given in a graduated manner according to the WHO pandemic phase so people know what needs to be done at each phase. You may have many people in your pandemic preparedness committee and the duties can be divided amongst them.  Make sure all personnel listed are trained to do the responsibilities assigned.
