What is Healthy Kids?
Healthy Kids is a Medicaid health care program for low-income
children under age 19 and for Pregnant Women of any age. There is an income
limit. There is no monthly premium for Healthy Kids. Contact the local
Department of Human Services in your county to apply for this program.
The limit on family income depends on your children's ages and on the size of
your family. Unborn children are included in family size.

Is There a Limit on Income?
Income guidelines are based on the Federal
Poverty Guidelines. The limit on family income depends on your children's ages and on the size of your family.

Is There a Limit on Assets?
No. Assets such as your home, savings
and checking accounts and cars are not counted.
Are There
Other Requirements?
You must meet the following requirements:
- Have a social security number for each person
requesting Healthy Kids. If someone does not have a social security number, we will help
them apply for one.
- Be a resident of Michigan.
- Have proof of your alien status if you are not a
U.S. citizen.
How Do I Apply For Healthy Kids?
You may apply for Healthy Kids at your local
Family Independence Agency (DHS) office. There is at least one DHS office in each
county. Applications are also available at the Marquette County Health Department. Contact
either office and ask for an application form.
If you just want Healthy Kids, ask for
DCH-0373-D.
If you or your family need help with food,
shelter or other daily living expenses, you need to call your local Family
Independence Agency and ask for DHS-1171.
You must file a signed application. You
may be asked to come to the local DHS office. Tell the worker if you need help with past
medical bills. Your coverage may begin three months before you apply.
After I Apply for Healthy Kids
What
Happens?
The local DHS office decides your
eligibility. (Applications taken at the local Public Health Department are forwarded
to your local DHS office). You are notified of the decision within 30 days.
If I or my Children are
Eligible for Healthy Kids, What Happens Next?
You receive a MIHealth
Card. You must present the card each time any of the eligible persons
listed on the card request medical services. The card may only be used for the
persons listed.
What if Things Change?
You must report to your DHS worker within 10
days any changes that may affect eligibility, such as:
- Pregnancy ends
- Income
- Employment
- Health insurance coverage
- Persons at home
- Address
- Any other change
What Does Healthy Kids Pay For?
This is a general listing of Medicaid covered
services that are available when medically necessary and furnished by enrolled providers.
Some services are only for certain age groups or may require prior approval.
The services listed below are subject to change.
Payment is made directly to the provider. You cannot be reimbursed directly.
You must use providers enrolled in Healthy Kids.
- Prenatal care and delivery services
- Maternal and infant support services for some
pregnant women and children
- Medicaid health screening for recipients under age
21
- Inpatient and outpatient hospital services
- Services of physicians (MD or DO)
- Laboratory and x-ray services
- Family planning services
- Home health services
- Visit services (limited)
- Dental services (limited)
- Hearing services, including hearing aids (limited)
- Community mental services
- Pharmacy services and medical supplies
- Therapy (occupational, physical, speech)
- Treatment for drug addiction and alcoholism
- Methadone maintenance
- Ambulance services (limited).
Contact your local
DHS office about other transportation to a
Medicaid covered service.
Fraud
State and Federal laws have penalties for a person found guilty of fraud. Misusing
your Medicaid card, making false statements, or failing to report changes is against the
law. If evidence indicates willful violations of law, law enforcement officials will
investigate. Anyone who aids another person to obtain Medicaid fraudulently is
subject to the same penalties.
